Suffolk County Government is seeking an Intensive Case Manager for children and youth to provide intensive case management services in a mental health setting. The role involves planning, problem-solving, and coordinating services for clients and their families, with a focus on overcoming obstacles to access necessary resources

Job Summary

  • Responsible for helping youth and their family access needed services, including service planning, child and family team meetings, referrals and access to needed services, advocacy, and coordinate and manage services across system.
  • Maintain regular contact with clients and collateral contacts providing face-to-face visits in the home, school and community to provide crisis prevention/intervention/stabilization, referral and advocacy services, coordination of services and assistance with basic needs, as well as engage in discharge planning with youth and family.
