Job Summary
Provide physical therapy treatments under the supervision and direction of the Physical Therapist (PT), ensuring all interventions follow the participant’s individualized care plan.
Conduct scheduled home visits for treatments, mobility assessments, and check-ins, ensuring continuity of care for participants in their residence or alternative care settings.
