  • The role provides oversight on weekly PDPM meetings and monitors facility utilization of PDPM tools to ensure optimum outcomes.
  • The incumbent is responsible for assessing health status, determining level of care, and ensuring timely completion of all MDS assessments including PPS Medicare.
  • This position requires coordinating interdisciplinary participation in completing the Minimum Data Set according to strict regulatory time frames.
