The primary purpose of this position is to provide each assigned resident with routine daily care and services in accordance with their assessment and care plan.
Employees must report all changes in resident condition, accidents, incidents, and suspected abuse immediately to the Health and Wellness Director or Executive Director.
The role requires assisting residents with personal functions such as bathing, dressing, feeding, and mobility while maintaining strict confidentiality of protected health information.

Skills & Requirements
Must-have
Assist residents with daily hygiene and dressing
Monitor and report changes in resident condition
Maintain confidentiality of protected health information
Lift and move residents up to 25 pounds
Record intake, output, and vital signs data

Create warm and positive atmosphere for residents
Cooperate effectively with nursing personnel
Demonstrate patience with disoriented residents
Follow established safety policies strictly
Key Requirements
High school education or equivalent required
Proficiency in Microsoft Suite products required
