  • Each day, providers conduct six in-home visits alongside one virtual consultation, ensuring accessibility and continuity of care for Medicare patients.

Salary

Base: $56.76 - $87.59 per hour; Bonus/Equity: $5,000 sign-on bonus or $7,500 relocation assistance; Benefits: $12,000 travel stipend plus comprehensive benefits package

Key Requirements

  • APRN or PA-C degree
  • Board Certification in relevant discipline
  • Minimum two years primary care experience
  • Valid driver’s license and insurance
  • Successful credentialing with Intermountain Health
