  • Coordinate care with residents, care providers, financial services, and third party payers while overseeing all admissions and discharge activities.
  • Must have excellent time management skills to develop organized work processes in a high volume environment with rapidly changing priorities.

Key Requirements

  • Bachelor’s Degree in Nursing or Social Work
  • Registered Nurse (RN) license preferred
  • Licensed LVN or LPN nurse acceptable
  • Two years clinical nursing experience preferred
  • Knowledge of Medicare, Medicaid programs and benefits
  • Valid LVN/LPN or RN license in state of employment
  • Certified Case Manager (CCM) certificate a plus
  • Maintain required continuing education and licensing
