The Restorative Nursing Assistant provides restorative nursing services that assist the resident to restore, improve or maintain bodily functions to the highest degree practicable in accordance with the resident's assessment, care plan and as directed by supervisors.
Documents objective information and care provided to the resident per facility charting guidelines, including daily charting of services performed and weekly summary of resident progress with restorative services.
Reports observations and pertinent information regarding changes in resident functioning, behavior, mood, physical condition or refusal of services to Supervisor and provides input regarding plan of care based on past experience.
