Job Summary
Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility effective utilization of PDPM tools.
Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change.
Responsible for all level of care changes within the facility and maintains confidentiality of resident care information.
