Job Summary
Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility effective utilization of PDPM tools.
Ensures the accurate and timely completion of all MDS Assessments, coordinates interdisciplinary participation, and maintains an accurate schedule of all MDS assessments.
Responsible for all level of care changes within the facility, notifies all departments, and generates appropriate forms for acuity changes.
