Assist residents in achieving the highest practicable level of self-care, independence and well-being by providing medically related social services.
Evaluate social and family information to assess social services needs and develop care plans, including providing emotional support and assisting with healthcare decisions.
Develop and maintain strong working relationships with internal departments and external community agencies to meet resident needs, including assisting with discharge planning and securing resources.
