  • Assesses readmitted patients for perceived reasons
  • Organizes and facilitates patient and family care conferences
  • Documents discharge planning evaluation and needs
  • Communicates with Payors for authorization
  • Assesses patients and families holistically for discharge planning
  • Reviews medical record for transition of care plan
  • Develops discharge plans with contingency plans
  • Leverages technology for communication with post-acute care services

Key Requirements

  • Associate's of Nursing
  • 2+ medical/hospital nursing experience
  • Registered Nurse (RN)
  • Prior Care Management/Utilization Management experience
