Provides medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident, including emotional support and coping assistance.
Assists residents with health care decisions, personal shopping, inventory tracking, and coordinates responses to missing belongings, while also securing community resources and assisting with discharge planning.
