Job Summary

  • This position is responsible for the coordination of the entire transplant process from day of referral to time of transplant for potential heart and lung transplant candidates.
  • The role serves as a liaison for the transplant team with pulmonologists, cardiologists, consulting services, dietetics, social services and insurance company representatives.
  • Candidates must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age and acuity of the patients being served.
