Navicent Health Medical Center is seeking a part-time Care Manager Social Worker to coordinate patient care and discharge planning. The role requires a Bachelor's degree in Social Work and a minimum of three years of relevant experience in acute care or care management
Job Summary
Assesses patients to determine their discharge planning and/or post-acute transition needs and develops the discharge plan utilizing internal and external resources to ensure a safe discharge or transition to alternate level of care.
Works collaboratively with patient's physicians and members of the multidisciplinary team to assure communication and exchange of input related to patient's specific care needs, utilizing clinical judgment and evidence-based guidelines.
Advocates in the patient's best interest for necessary funding, treatment alternatives, timelines and coordination of care, with frequent evaluations of progress and goals, and identifies community and caregiver resources to ensure continuity of care.
