This role is responsible for assisting in the planning, developing, and directing of social service programs to meet the medically related emotional and social needs of residents.
The incumbent must assist residents in achieving the highest practicable level of self-care, independence, and well-being while coordinating activities with other departments.
The position requires maintaining confidentiality of all resident care information and reporting suspected incidents of fraud or unauthorized disclosure.

Key Requirements
Bachelor's Degree in Social Work (BSW)
One year supervised social work experience in healthcare
State social work licensure if required by law
Completion of mandatory in-service trainings including HIPAA
Prior long-term care or skilled nursing facility experience preferred
