Job Summary
The Community Health Coordinator IV serves as the primary case manager for the UofL 550 Clinic, supporting people living with HIV/AIDS (PLWH/A) in accessing medical care, essential support services, and long-term retention in care.
Responsibilities include conducting initial assessments, maintaining patient eligibility records, managing intake and enrollment, and making timely referrals to Part B and other supportive services to ensure continuity of care.
Build and sustain relationships with community partners and stakeholders to expand resource access and enhance patient outcomes.
