Job Summary

  • Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility utilization of PDPM tools.
  • Ensures accurate and timely completion of all MDS Assessments, coordinating interdisciplinary participation and maintaining documentation standards.
  • Responsible for all level of care changes within the facility, notifying departments and transmitting necessary forms.
