Job Summary
Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility utilization of PDPM tools.
Ensures accurate and timely completion of all MDS Assessments, coordinating interdisciplinary participation and maintaining documentation standards.
Responsible for all level of care changes within the facility, notifying departments and transmitting necessary forms.
