Job Summary
Provides psychosocial services to patients treated by the facility including in-center and home dialysis patients utilizing Social Work Theory of Human Behavior and accepted methods of social work practice.
Works with the health care team to promote positive adjustment, rehabilitation and improved quality of life for our patients while supporting Quality Indicators and Outcomes.
Participates in patient assessment, care planning, counseling, patient education, and quality improvement activities to ensure comprehensive quality care.
