  • The incumbent is an integral member of the Care Management team, focusing on the social determinants of health to improve care quality, patient experience, and population health.
  • Responsibilities include comprehensive assessments, implementing recommendations, collaborating on safe transition plans, educating patients and families, and providing crisis interventions for various sensitive issues.
