This job utilizes principles of care coordination to support patients and their families/caregivers, collaborating with an interdisciplinary team to advocate for patients and coordinate care across the continuum.
The incumbent is an integral member of the Care Management team, focusing on the social determinants of health to improve care quality, patient experience, and population health.
Responsibilities include comprehensive assessments, implementing recommendations, collaborating on safe transition plans, educating patients and families, and providing crisis interventions for various sensitive issues.
