  • The Community Outreach Worker provides a range of clinical services including screening, assessment, and case management to adults and families affected by mental illness and substance use.
  • This role requires a high degree of independent practice with the ability to implement an evidenced-based plan of care using principles of harm reduction and psychosocial rehabilitation.

Must-have

  • Bachelor of Social Work degree required
  • One year post BSW clinical experience in Mental Health
  • Screening and suicide risk assessment skills
  • Harm reduction and recovery-oriented practice knowledge
  • Motivational interviewing and group facilitation skills
