Must-have
BA or BS in Social Work required
One year case management experience
Experience with frail older adult population
Knowledge of PACE regulatory requirements
Strong communication with interdisciplinary teams
Nice-to-have
Bilingual in English/Cantonese or Spanish

Ethical standards adherence from NASW
Key Requirements
Bachelor's degree in Social Work or related field
