Key Requirements
Doctor of Pharmacy (PharmD)
Active Louisiana pharmacist license
Oncology Pharmacy Specialty Certification (BCOP) within 1 year
PGY1 + PGY2 residency or equivalent experience
3 years experience in hematology/oncology specialty
