  • The role involves fostering and facilitating the care management process, conducting comprehensive assessments, and collaborating cross-functionally to ensure success of care management implementation.

Key Requirements

  • MH/SU license or NC Substance Abuse Board certification
  • RN with 4 years MH/SU/IDD experience
  • Master’s or Bachelor’s degree in human service or related field with relevant experience
  • Care coordination, care management, or case management experience
  • Valid driver’s license
  • Clinical license preferred
  • Criminal background and registry checks
  • CPR proficiency
