Job Summary

  • Provides support to individuals with Mental Health (MH) or Substance Abuse Issues in residential, school, workplace and community settings.
  • Responsible for the development, implementation, monitoring and revision to the Person Centered Plan in conjunction with the interdisciplinary healthcare team.
