Job Summary
Case Managers work in collaboration with the clinical team to identify, coordinate, and secure community-based resources to support the Adolescent client’s ongoing treatment and recovery.
The case manager also facilitates the admission and discharge process, coordinates outpatient care, and maintains up-to-date knowledge of relevant community resources and the procedures for accessing services from external providers and social agencies.

Key Requirements
Bachelor's degree in mental health related field
