Job Summary
The Care Management Support Lead is responsible for coordinating with and bridging gaps between the market care management team and the enterprise/Highly Integrated Dual Eligible Special Needs Plan (HIDE SNP) care management team(s).
Collaborates with Population Health, Quality, and Clinical Leaders to reduce barriers to care, decrease health disparities, support at-risk, underserved, and rural communities, and address HRSNs that impact Enrollees’ health and well-being.

Base: $86,300 - $118,700 per year; Bonus/Equity: Eligible for bonus incentive plan; Benefits: Medical, dental, vision, 401(k), time off, disability, life insurance

Key Requirements
Bachelor’s degree in nursing (BSN) or social work
Active, unrestricted Michigan RN or LSW license
5+ years clinical experience (UM, Case Management, Managed Care)
2+ years leadership experience
Knowledge of Medicare/Medicaid regulations and NCQA Standards
Intermediate to advanced Microsoft Office proficiency
Experience with electronic case notes documentation
