Job Summary

  • The Care Management Support Lead is responsible for coordinating with and bridging gaps between the market care management team and the enterprise/Highly Integrated Dual Eligible Special Needs Plan (HIDE SNP) care management team(s).
  • Collaborates with Population Health, Quality, and Clinical Leaders to reduce barriers to care, decrease health disparities, support at-risk, underserved, and rural communities, and address HRSNs that impact Enrollees’ health and well-being.

Key Requirements

  • Bachelor’s degree in nursing (BSN) or social work
  • Active, unrestricted Michigan RN or LSW license
  • 5+ years clinical experience (UM, Case Management, Managed Care)
  • 2+ years leadership experience
  • Knowledge of Medicare/Medicaid regulations and NCQA Standards
  • Intermediate to advanced Microsoft Office proficiency
  • Experience with electronic case notes documentation
