** The University of Arkansas for Medical Sciences is seeking an RN Case Manager for Outpatient Care Management to provide longitudinal care management services for patients with chronic conditions. The ideal candidate should possess a nursing degree and at least three years of experience, preferably with care management experience in an ambulatory setting. **

Job Summary

  • The RN Care Manager provides longitudinal care management services, by phone and face to face, to promote wellness, improved outcomes and self-care within a defined population of patients with chronic condition(s), complex health needs, and support their appropriate utilization of health services.

Key Requirements

  • 3 years RN experience
  • Current BLS certification
  • Valid Arkansas Registered Nursing License
  • Baccalaureate Degree, ADN, or Diploma RN Degree
  • Certification in Case Management (CCM, ANCC) or obtain within 1 year
  • BSN preferred
  • Care Management experience in Ambulatory setting
