Job Summary
The Service Coordinator II supports maximizing the Member’s health, wellbeing, and independence by focusing on person-centered care planning and assessing service and support needs.
This role facilitates clinically appropriate and fiscally responsible patient care through communication with physicians, family, caregivers, and the healthcare team, addressing holistic needs.
The Service Coordinator II will be designated for Members with primarily a behavioral health diagnosis, coordinating behavioral health services and completing assessments like the STAR Kids Screening and Assessment Instrument (SAI).
