Job Summary
The role provides oversight on facilities' weekly PDPM meetings for skilled patients and tracks effective utilization of PDPM tools.
The position ensures accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change assessments.
The employee coordinates interdisciplinary participation in completing the Minimum Data Set for each resident according to regulatory time frames.

Skills & Requirements
Must-have
Accredited School of Nursing graduation
State Board of Nursing good standing
PDPM tools and MDS assessment completion
Interdisciplinary care conference coordination
Federal and state regulatory compliance
Nice-to-have
Bachelor of Science Degree in Nursing preferred
3 years supervisory experience preferred
Long term care experience preferred
PCC software knowledge preferred
Ability to incorporate new methods
Key Requirements
