Job Summary
The Community Rehabilitation Team (CRT) West supports people living with complex mental health problems to achieve their goals, along with gaining the skills and confidence to live as independently as possible.
This role would sit within our mental health complex care services in the Dorset Council area; this incorporates Community Rehab Team, which is based at Forston Clinic, Assertive Outreach Team which is based in Weymouth and the Glendinning Unit in Dorchester.
You will need to work creatively to deliver a high standard of care that optimises a person’s own health, wellbeing and recovery.
