  • The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput and to enhance care quality and efficiency and ensure efficient resource utilization.
  • The PCRM collaborates with physicians, nurses and other health care workers to manage the patient transition to the next level of care.
