Job Summary

• This position is responsible for delivering patient-centred care within the WA Home Therapies setting in accordance with organisational core values and APHRA standards.
• The role involves training patients and carers to manage either peritoneal dialysis or haemodialysis while functioning as the primary coordinator of care for a group of dialysis patients.
• The incumbent must maintain accurate patient records, participate in quality improvement processes, and ensure a safe working environment by adhering to Occupational Health and Safety plans.
