Job Summary
The role involves facilitating orderly transitions for patients from Med/Surg and ICU settings to the next appropriate level of care while working closely with physicians and payors.
Case managers serve as liaisons to ensure continuity of care by providing medical updates to outside facilities and coordinating complex discharges to various care settings including skilled nursing and hospice.
