Job Summary
Assess and evaluate members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families towards and facilitate interaction with resources appropriate for the care and wellbeing of members.
Visits Medicaid members in their homes, Assisted Living Facilities (ALFs), and/or Long-Term Care Facilities and other care settings – 75-90% local travel.

Key Requirements
Bachelor's degree in social sciences, social work, human services, or related field
Active, unrestricted LPN in Illinois with 1 year experience
Reside in Cook, Lake or Mchenry County, IL
1+ year experience in health care and/or case management
1+ year experience with Medicare and Medicaid recipients
