Job Summary
The Care Manager-Registered Nurse (RN) communicates and coordinates health care services for participants in the Program for All Inclusive Care for the Elderly (PACE).
Provides evidence based comprehensive care coordination, monitoring, support, health education and coordination of services in the home or PACE Clinic setting.
Collaborates and coordinates with the participant, significant others, physician, pharmacist, and other providers in holistic health care coordination, planning and care management provision of care across settings.
