Job Summary

  • Regularly communicate with residents and their families about Plans of Care, PT, OT and other treatment protocols.
  • Monitor and adjust resident’s statuses based on changing needs and conditions, and organize and prioritize daily work by assessing resident needs.
  • Coordinate care with resident, care providers, facilities financial services, and third party payers, and oversee all admissions and discharge activities.

Key Requirements

  • Registered Nurse (RN) license preferred

  • LVN/LPN Valid License or RN Valid Licensee
