Job Summary
Communicate regularly with residents and their family about Plans of Care, PT, OT and other treatment protocols to ensure they feel safe and secure in our care.
Oversee all admissions and discharge activities while coordinating care with residents, care providers, financial services, and third party payers.
Must have excellent time management skills to develop organized work processes in a high volume environment with rapidly changing priorities.

Key Requirements
Bachelor’s Degree in Nursing or Social Work
Registered Nurse (RN) license preferred
Licensed LVN or LPN nurse acceptable
Two years clinical nursing experience preferred
Knowledge of Medicare, Medicaid programs
Valid LVN/LPN or RN license in state of work
Certified Case Manager (CCM) certificate a plus
Maintain required continuing education and licensing
