Job Summary
Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility effective utilization of PDPM tools.
Tracks and monitors QM and QASP performance monthly and quarterly, providing data analysis and clinical recommendations for optimum outcomes.
Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change, and coordinates interdisciplinary participation.
