The Social Worker provides comprehensive psychosocial, emotional, spiritual, and bereavement support to Hospice patients and their families.
This role requires conducting initial assessments, developing goal-directed plans of care, and providing crisis intervention as an integral member of the Interdisciplinary Group.
Candidates must possess a valid driver's license, reliable transportation, and current auto liability insurance to perform essential job functions.

Must-have
Active LMSW credential or MSW degree
One year healthcare experience required
Valid driver's license and reliable transportation

Recent active patient contact within three years
Adaptability to diverse cultural backgrounds
Emotional stability for stress management

Key Requirements
Active LMSW credential OR MSW with one year healthcare experience
BSW with one year healthcare experience under MSW supervision
Bachelor's degree in psychology/sociology with supervised social work experience
Minimum two years social work experience preferred
Active patient contact within past three years preferred
