Responsible for providing engagement and service brokering to previously homeless individuals experiencing substance use disorder.
Engaging them in basic conversation, assessing their needs, and connecting them to community resources, with an emphasis on assisting individuals maintain their housing.
Work as a member of the team to identify service needs and options and assist member in identifying and accessing needed services and resources.
