  • The Per Diem RN Care Manager will participate in a dynamic interdisciplinary team to provide case management services to identified complex, high-risk patients across the care continuum and partner with patients and families to improve patient experience.

Key Requirements

  • Bachelor’s Degree required
  • Valid OR RN License required
  • 2 years care manager experience or 5 years nursing experience
  • Valid Driver’s License required
  • Certified Case Manager preferred
  • Certified Diabetes Educator preferred
