MTC Care is seeking an experienced full-time PCM Care Coordinator to perform remote monitoring and principal care management for patients in skilled nursing and assisted living facilities.
The role requires gathering medical information between visits, reporting monthly concerns such as falls and medication issues, and ensuring all interventions are completed for approximately 15 patients per day.
Candidates must be located in Mississippi, Alabama, Ohio, Iowa, Missouri, Pennsylvania, or West Virginia and work EST hours.
