The University of Arkansas for Medical Sciences is seeking an RN Case Manager for Outpatient Care Management to provide longitudinal care management services to patients with chronic conditions. The ideal candidate will have a nursing degree, experience in care management, and a commitment to improving patient outcomes

Job Summary

  • The RN Care Manager provides longitudinal care management services, by phone and face to face, to promote wellness, improved outcomes and self-care within a defined population of patients with chronic condition(s), complex health needs, and support their appropriate utilization of health services.

Key Requirements

  • Baccalaureate Degree, ADN, or Diploma RN Degree
  • 3 years RN experience
  • current BLS certification
  • Valid Arkansas Registered Nursing License
  • Certification in Case Management (CCM, ANCC) or obtain within 1 year
  • BSN preferred
  • Care Management experience in Ambulatory setting
