Job Summary
Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks PDPM tools utilization.
Ensures accurate and timely completion of all MDS Assessments, coordinates interdisciplinary participation, and maintains an accurate schedule of assessments.
Schedules and conducts resident care conferences, assists disciplines in formulating and revising care plans, and is responsible for all level of care changes within the facility.
