Key Requirements

  • Master’s Degree in Nursing or Physician Assistant
  • Psychiatric/Mental Health emphasis
  • Certified as a Psychiatric Mental Health Nurse Practitioner or Physician Assistant
  • Active Psychiatric Mental Health Nurse Practitioner or Physician Assistant State License
  • State CDS Certification and DEA License
  • NPI license
  • Minimum of two (2) years of experience working with the frail or elderly
  • Reliable means of transportation
