The primary purpose of this position is to assist in planning and directing social service programs to meet the medically related emotional and social needs of residents.
Employees must coordinate social service activities with other departments and assist residents in achieving the highest practicable level of self-care and independence.
The role requires maintaining confidentiality of all resident care information and reporting any suspected incidents of fraud or unauthorized disclosure.

Skills & Requirements
Must-have
High school diploma or equivalent required
Ability to read and interpret regulations
Experience with Medicare/Medicaid assistance
Strong written and verbal communication skills
Proficiency in maintaining confidential records
Nice-to-have
Bachelor's degree in related field preferred
Prior healthcare experience preferred
Ability to work collaboratively with medical staff
