The clinician provides intensive care management for adults with serious mental illness admitted to or discharged from Harding Hospital's inpatient units.
This role ensures comprehensive medication oversight including REMS-required laboratory monitoring and coordinates transitions of care to reduce emergency department utilization.
The position requires high levels of community engagement before, during, and after hospitalization to promote treatment adherence and address social determinants of health.
