Skills & Requirements
Must-have
Bilingual Spanish proficiency required
CHW certification or willingness to obtain
3-5 years Community Health Worker experience
1 year care coordination experience
Valid driver's license and personal vehicle
Knowledge of MassHealth and ACA programs
Nice-to-have
Bachelor's degree in allied health discipline
Experience with Medicare/Medicaid populations
Motivational interviewing skills
Electronic Medical Record proficiency
Medical terminology knowledge
Key Requirements
Board of Certification of Community Health Workers credential
Minimum 3-5 years CHW experience in healthcare
Valid driver's license with safe driving record
Personal vehicle with current insurance
Willingness to obtain certification within one year if not held
