This position will train at our 525 E Mound Street, Columbus location and will work in the community supporting the following counties: Belmont, Coshocton, Guernsey, Harrison, Jefferson, Morgan, Monroe, Muskingum, Noble, Perry, Washington.
The Case Management Extender PFK helps navigate and access community services and other resources and provides support through maintaining population health programs and care coordination activities.
Case Management Extenders are patient and family facing with daily activities during inpatient stays, outpatient specialty clinic visits, emergency room visits, home, community visits, and via telephone.

Key Requirements
Bachelor's degree in health care, public health, or related clinical field, preferred
Valid Ohio driver's license and proof of auto insurance
Active BLS certification, required
