The Community Health Worker will primarily be working with families and school partners from the Joplin community to improve access to primary, medical, dental, and vision care for Score 1 for Health children.
Essential duties include providing support, advocacy, and education to parents and children, coaching parents in effective health care management, and documenting activities according to policies.
The role requires working closely with medical providers, social service agencies, and building positive relationships to facilitate communication and coordinate services.
